I have a local Ambulance District that has been giving their (Volunteer) President of the Board a check for $100 each month. Shouldn’t that be run through payroll? And what are the rules for volunteer board members?

Bottom line: The money is income, but not payroll. The board member would get a 1099-MISC at the end of the year and pay taxes on any amount after allowed deductions.
