Application fees are:

  • Theater capacity of 1 to 499 seats:
    – Initial Registration Fee – $200.00
    – Renewal Fee – $200.00
  • All others:
    – Initial Registration Fee – $350.00
    – Renewal Fee – $200.00
  • Group Employer – $200.00
